NaviCare is a free online navigation centre for children with complex care needs under the age of 25 in New Brunswick. The patient navigator helps families and care providers navigate the health, social and education systems in NB. We can help by coordinating patient care, facilitating transitions, connecting families with resources, informing families about the health, education, and social services available to meet their needs, and being a resource for the team. Our vision is that every child/youth with complex care needs has access to the health, social, and education services that he or she requires in a timely manner. The aim of NaviCare/SoinsNavi is to help facilitate more convenient and integrated care to support the physical, mental, emotional, social, cultural and spiritual needs of children/youth and their families using a personalized family-centred model of care.
